About the Role:
As a Home Health Registered Nurse, you will play a vital role in providing comprehensive and compassionate care to patients in the comfort of their own homes. You will work closely with patients, families, and other healthcare professionals to develop and implement individualized care plans, assess patient needs, administer medications, and provide education and support.
Home Health Registered Nurse Responsibilities:
* Perform basic nursing assessments and develop individualized care plans
* Administer medications and treatments as prescribed
* Monitor patient vital signs and symptoms
* Educate patients and families about their conditions and treatment plans
* Collaborate with physicians, therapists, and other healthcare professionals
* Document care provided in accordance with agency policies and procedures
* Maintain a clean and safe environment for patients
* Promote patient independence and quality of life
Home Health Registered Nurse Qualifications:
* Current RN license in the state
* Minimum of 1 year of experience in home health or a related setting
* Strong assessment, communication, and interpersonal skills
* Compassionate and patient-centered approach to care
* Ability to work independently and as part of a team
* Excellent time management and organizational skills
* Proficient in electronic medical records systems
* Valid driver's license and reliable transportation
