FUNCTION: The Medical Records/Health Information Coordinator assumes, responsibility and accountability for the record keeping procedures and storage of all clinical records, in a manner consistent with community policies and procedures, professional standards and state and federal laws and regulations for long term care facilities. Establishes and implements policies to ensure that records are complete, accurately documented, readily accessible and systematically organized. Collaborates with Administrator or Director of Nursing to allocate department resources in an efficient and economic manner to achieve department objectives. QUALIFICATIONS: The requirements listed below are representative of the knowledge, skill and/or ability required. Minimum Qualifications: Education and/or Experience: High school or equivalent is required. A background in the use of computerized systems is preferred. Excellent typing, communication and time management skills are essential. Ability to move from one task to another effectively. Proven ability to work effectively in a changing environment. REASONING ABILITY: Ability to define and solve problems collect data, establish facts, and draw valid conclusions. Ability to interpret and extensive variety of technical instructions in statistical or diagram form and deal with several abstract and concrete variables. Job Description ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Prepares charts for new admissions.
  • Consults with staff to help define and analyze information needed from the clinical record.
  • Develops a schedule for audits of the clinical record. • Conducts audits of the clinical record as scheduled.
  • Maintains the computerized physician order program and monitors physician visit schedule.
  • Print computerized reports as required. • Assists as required in communications with appropriate staff to facilitate completion and accuracy of the clinical record.
  • Develops and maintains a tracking system to ensure telephone orders are signed promptly.
  • Completes sections of the Minimum Data Set as required.
  • Prints completed MOS, RAP Summary, RAP Reviews and Care Plans according to schedule.
  • Processes transfer or discharge records and maintains appropriate clinical records on former residents.
  • Conducts clinical record destruction according to regulation and policy.
  • Prepares clinical records for legal proceedings.
  • Enters or records data timely and accurately.
  • Protects confidentiality of resident information.
  • Protects data against loss or destruction.
  • Reports suspected violation of security/confidentiality issues.
Receive and follow work schedule/instructions from supervisor and as outlined in established policies and procedures.
  • Assist in organizing and planning the medical records section in accordance with established policies and procedures.
  • Assist in the orientation program for all personnel in accordance with policies and procedures.
MEDICAL RECORDS/HEAL TH INFORMATION COORDINATOR Job Description
  • Assist the Medical Records Consultant as required.
  • Maintain minutes of meetings/files, as necessary.
  • Develop and maintain a good working rapport with inter-department personnel, as well as other departments within the community, to ensure that medical records can be properly maintained.
  • Ensure incomplete records/charts are returned to appropriate department for correction.
  • Assist in developing procedures to ensure records are properly completed, coded, signed, indexed, etc., before filing.
  • Ensure accurate and complete diagnosis coding as assigned.
  • Follow procedures to ensure charts/records do not leave the section except as authorized in our policies and procedures.
  • Maintain a record of authorized information taken from charts/records, (i.e., type information, name of recipient, date, department, etc.).
  • Abstract information from records as authorized/required for insurance companies, Medicare, Medicaid, VA, etc.
  • Index medical records as directed by the medical records consultant.
  • Maintain various registries as directed including register for admission and discharge of residents.
  • Collect charts, assemble them in proper order, and inspect them for completion.
  • Perform miscellaneous duties pertaining to medical records and assist business office personnel as required/directed.
  • Answer telephone inquiries concerning medical records functions. Prepare written correspondence, as necessary.
  • Retrieve medical records when requested by authorized personnel (i.e., physicians, nurses, government agencies and personnel, etc.).
  • Assure that medical records taken from the department are signed out and signed upon return to the department.
  • File active and inactive records in accordance with established policies. Committee Functions:
  • Perform support duties for committees of the community as directed. • Collect and assemble/compile records for committee review, as requested, and prepare reports for staff/other committees as directed.
