  • Call patients regarding medication adherence
  • Uncover challenges related to medication adherence
  • Provide patients with suggestions and resources on how to improve medication adherence

Key Responsibilities
  • Contact pharmacies, physicians, and medical groups as needed to help improve overall care of the patient related to medication adherence.

Key Responsibilities
  • Document call success rates and escalations within the provided excel template
  • Document successful calls within Practice Fusion EHR
  • Sign off on one’s own documented calls within Practice Fusion

Attend training sessions and call center meetings.
