Triage Nurse Endocrinology The purpose of this position is to improve the patient experience and clinical outcomes of the Endocrinology Service by improving access to care, ensuring smooth transitions between inpatient and outpatient care and providing effective chronic illness care of Endocrine disorders. Middleton Family Medicine is dedicated to providing a 'medical home' model in our specialty practices, including advanced access (telephone, Athena Medical Records, electronic consults, sick visits). We recognize the need for long-term management in specialty practices for patients with chronic illness. In the out-patient setting, we are committed to excellent Endocrine management in the acute care context which requires multidisciplinary coordination. The RN works under the clinical supervision of our Endocrinologist, Lead for Endocrine Department, M.Ed., RD, CDCES and FNP/CDCES and are responsible for training and ensuring quality of care. RESPONSIBILITIES: Specific functions include but are not limited to:
- Providing telephone-based management for Endocrinology patients calling with minor concerns based on published protocols.
- Review and triage electronic consultation requests; including providing management to referring physicians & determining which patients should be evaluated in the Endocrine department
- Coordinate care of patients referred to the Endocrinology service
- Help to coordinate scheduling of Endocrinology hormonal/metabolic testing, biopsies and procedures.
- Perform pre-procedure or testing evaluations including history and physicals and obtaining informed consent including those directly referred.
- Perform ambulatory procedures including but not limited to ACTH stimulation testing.

- Ability to learn use and interpretation of Continuous glucose monitoring and assist with insulin pump therapy.
Compensation: $30/hr-$40/hr
