Salary Range: $90,700.00 - $103,000.00
*As required under NYC Human Rights Law Int 1208-2018 - Salary range for this role when Hired for NYC Offices
Position Summary Provides quality diabetes/nutrition care and education to patients and family members. This position assesses the diabetic/nutritional status of patients and recommends the appropriate diabetic/nutrition therapy as it pertains to the patients' diabetic/nutritional needs.
Job Responsibilities - Applies knowledge of diabetic/nutrition screening and assessment to specific patient populations to determine their diabetic/nutritional needs. Provides education in diabetes technology, including pumps, continuous glucose monitoring.
- Develops educational material on nutrition, diabetes, weight management, and other nutrition-related concerns. Actively participates in overall education and training activities, and disseminates information regarding current nutrition practices.
- Evaluates patients' nutritional status by reviewing pertinent information obtained from the patient, medical records, and the interdisciplinary health care team.
- Provides nutrition counseling to patients and family members as indicated.
- Uses professional judgment to adapt and apply guidelines, protocols and professional standards of care to the new and changing needs of the patient. Evaluates nutrition care plans for effectiveness and recommends changes as needed.
- Performs other duties as assigned.
Education Experience Master's degree is preferred.
Approximately 3-5 years of related experience.
Knowledge, Skills and Abilities - Good interpersonal skills in order to develop working relationships with care center staff and other personnel.
- Good computer skills to develop group education materials.
- Ability to conduct workshops on nutrition-related topics.
Licenses and Certifications - Specialty Credential - Certified Diabetes Educator (CDE)
- Certified Dietitian Nutritionist (CDN) from NYS Office of the Professions
- Registered Dietitian (RD) with the Commission of the Academy of Nutrition and Dietetics
