Position Overview
This fully remote position supports patients and medical practices in the Atlanta, GA area. We are looking for an ideal candidate located in the Eastern or Central time zones.
In this role, you will collaborate with clinical teams to strengthen the connection between patients and their primary care physicians or medical practice staff, while improving overall patient health and well-being. You will engage with patients to help them navigate their care, resolve healthcare issues, and enhance communication with their medical practice.
Key Responsibilities
Collaborate remotely with medical practice staff and case management teams to manage a targeted patient population.
Function independently while being a highly collaborative team member and extension of medical practices.
Assist patients in achieving better health outcomes by addressing their questions, connecting them with their medical practice, and implementing high-impact care plans.
Accurately document patients' medical histories and issues in electronic health records (EHR).
Understand the referral and prior authorization processes.
Build and maintain trusting, professional relationships with assigned medical practices and within the organization.
Meet assigned Key Performance Indicators (KPIs) to ensure positive patient health outcomes while managing healthcare costs.
Utilize effective communication methods (calls, texts, portals, emails) to engage high-risk patients.
Appropriately escalate medical issues to the Health Guide Registered Nurse, primary care physician, or organizational leadership.
Travel occasionally to support primary care physician offices.
Adhere to all security policies and procedures to protect all Protected Health Information (PHI) and intellectual property.
Qualifications
Must be bilingual (Korean/English); Qualified Medical Interpreter is preferred.
Passion for patient coordination, especially for those with complex medical and social needs.
Empathy and a willingness to develop skills in motivational interviewing, critical listening, and behavioral change management.
Exceptional conversation skills and the ability to accurately document patient interactions in real-time using software.
Familiarity with medical terminology and the function of a primary care practice.
Understanding of social determinants of health.
Certification as a Medical Assistant or Emergency Medical Technician is preferred.
Experience with electronic health records (EHRs) and care management analytics.
Ability to manage competing priorities while remaining detail-oriented.
Proficiency with various EHR systems (e.g., EPIC, Athena, Cerner).
Strong written, visual, and verbal communication skills.
Ability to track multiple tasks and ensure their timely completion.
Comfortable working independently as part of a patient care team under licensed clinical supervision.
Proven success in a remote work environment.

Compensation
The hourly pay range for this role is between $20 and $23, paid bi-weekly. Final pay decisions are based on various factors, including skill set, relevant experience, education, location, and certifications.
