Position Overview:
As a Patient Care Coordinator, you will play a vital role in enhancing the connection between patients and their primary care physicians, while actively supporting their health and well-being. You will engage with patients to help them navigate their healthcare journeys, resolve issues, and foster effective communication with their medical practices.
What You'll Be Doing:
Collaborate primarily in a remote capacity with the patient's medical practice staff and our dedicated Case Management team to manage a targeted population of patients.
Operate independently while also being a key collaborative team member, serving as an extension of the medical practice.
Support patients in achieving better health outcomes by addressing their questions and concerns, connecting them with their medical practice, and partnering to implement impactful care plans.
Identify and close care gaps, escalate medical issues, and assist patients in accessing the right resources, including referrals to specialty care and community-based organizations (CBOs).
Document patients' medical histories and issues accurately in the electronic health record (EHR) system.
Navigate the referral and prior authorization processes effectively.
Build and maintain trusting, professional relationships with assigned medical practices and within our organization.
Meet assigned Key Performance Indicators (KPIs) to promote positive patient health outcomes and reduce medical spending.
Utilize various communication methods (calls, texts, portals, emails) to engage high-risk patients effectively.
Escalate medical issues appropriately to the Health Guide Registered Nurse, Primary Care Physician, or leadership.
Occasionally travel to supported primary care physician offices as needed.
Follow security policies and procedures to protect all Protected Health Information (PHI) and company data.
What You'll Need to Have:
Bilingual (Korean/English) - Native or Fluent required.
Certified or Registered Medical Assistant or certified EMT required.
A passion for patient coordination and serving individuals with complex medical and social needs.
Empathy and a desire to become proficient in motivational interviewing, critical listening, and behavioral change management for improved patient engagement.
Exceptional communication skills, with the ability to document patient interactions accurately and in real time.
Familiarity with medical terminology and the function of primary care practices.
Understanding of social determinants of health.
Proficient in using electronic health records (EHRs), care management analytics databases, and various software programs.
Strong ability to balance competing priorities while remaining detail-oriented.
Experience with electronic health records such as EPIC, Athena, and Cerner is a plus.
Excellent written, visual, and verbal communication skills.
Capability to track progress on multiple tasks and ensure timely completion.
Ability to work independently as part of a patient's overall care team, under the supervision of licensed clinical professionals.
Demonstrated success in a remote work environment.
Compensation:
The base pay range for this role is $24+ per hour, paid bi-weekly. Final compensation will depend on various factors, including skills, experience, education, location, and licensure/certifications.
